An exciting and unique opportunity has arisen to be a part of the Mental Health Intensive Support Team working within their Rehab Access Function supporting the Clinical lead in the coordination of rehabilitation and out of area assessments.
The role will involve formulating specialist assessments for individuals that require rehabilitation for both community and inpatient settings. You may be asked to deputise for the Clinical Lead during periods of annual leave, sickness and absence. In doing so you will act as a point of contact for all internal and external agencies associated with the running of the Rehab Access function.
In partnership with the Clinical Lead you will be responsible for screening referrals, liaising with professionals and families/carers. Completing assessments to formulate a Rehab pathway.
As part of this role you will liaise with Inpatient services within acute care to offer specialist advise around rehabilitation and support acute care in formulating discharge pathways.
Be able to signpost to the appropriate service to meet individual service user and carer needs. To deliver evidence based interventions which are measurable and which promote the service users journey along the recovery pathway.
You will be required to attend weekly Gatekeeping meetings where assessments are discussed with the Rehab MDT and external funding bodies.
Responsibility to supervise junior staff.
