About The Role
This is an exciting opportunity to be part of our Commissioning Team as a Transition Co-ordinator & Tracking Officer.
As our new Transition Co-ordinator & Tracking Officer, you’ll be the hero guiding young people with complex health and social care needs through their transition to adulthood. You'll work closely with SEND, attend Year 9+ reviews, and ensure seamless information flow between children’s and adult services. Plus, you'll be the mastermind behind our Transition Data Base, collecting and managing vital data to make informed decisions and support our young people effectively.

About The Team You’ll Be Working In
- Year 9+ Reviews: Attend crucial reviews to ensure every young
person’s needs are well-documented and communicated.
- Data Coordination: Maintain and update our Transition Tracking Database, gathering and inputting essential information from a range of sources.
- Stakeholder Engagement: Assist young people and their families by offering support and advice, ensuring effective communication and understanding.
- Collaborative Partner: Work closely with Children’s and Adults’ Services, the CCG, and Joint Commissioning to maximise resources and support.
- Diversity Champion: Celebrate and promote the rich diversity of Hounslow’s communities, ensuring our services are inclusive and equitable.
You will be working across Childrens and Adults services, reporting directly to the Complex tracking Manager.
About You
- You are committed to improving the lives of young people with disabilities and their parents and/or carer’s.
- You understand the range of inequalities experienced by young people with disabilities and can advocate on their behalf.
- You have great communication skills; you engage effectively with a broad range of people and always maintain confidentiality.
- You have a proactive, flexible, responsive approach, always seeking to provide the highest levels of customer service.
- You can work autonomously and creatively, generating new ideas to improve service delivery.
- An ability to process and understand key legislation and duties of the local authority.
