An exciting opportunity for two motivated mental health practitioners, one based in Cheshire West and one in Wirral (expectation to also cover across locality) who has experience of working within mental health services. If you are passionate about supporting people to further their recover, and to ensure people only stay in hospital as long as they need to - this is the role for you. We are seeking someone with a can do attitude, to join our Discharge Facilitation team, working alongside Patient Flow. The Discharge Coordinator role is a crucial role across acute care services, supporting to solve daily challenges and remove barriers to discharge. The post holder will work closely with community care coordinators, First
Response Services and inpatient teams, identifying any barriers to
discharge for CWP inpatients. They will work ‘hand in hand’ with their
colleagues in the team, other community teams in and out of Trust,
primary care colleagues and the patient’s family/carers to facilitate safe
and timely discharges. They will ensure that the estimated discharge date
as identified during reviews is progressed where possible and feasible.
The Post holder will be expected to work as part of the Urgent Care Mental Health First response service which includes Crisis Triage and response
services, Liaison Psychiatry, Home Treatment teams, Street Triage and
Liaison and Diversion Services
